As the operator was pushing the safety cable down, one of the uprights holding up the safety cable detached from the angle iron causing the operator to fall approximately 30 feet. The employee suffered a fractured pelvis and liver damage.

An employee was using a band saw to cut angle iron. One of the five pieces of angle iron that was being cut was shorter than the others and was not secured by the clamp.
